Bilateral

  • Botulism.

  • Myasthenia gravis.

  • Wernicke's encephalopathy.

  • Acute cranial polyneuropathy.

  • Brainstem stroke.

Unilateral

  • Carotid-posterior  (3rd  cranial  nerve,  pupil  involved  communicating aneurysm).

  • Diabetic-idiopathic (3rd or 6th cranial nerve, pupil spared).

  • Myasthenia gravis.

  • Brainstem stroke.
